Each member is responsible for maximizing their benefits by scheduling appointments during the 12 month membership period. Refunds are not granted if benefits are not utilized.

Any Exclusions?

This membership plan is only valid at Eastridge Dental. This is NOT an insurance plan.

This membership is non-transferrable.

Products such as whitening, refills, electronic toothbrushes, Peridex, or Prevident are not included.

Membership discounts are not applicable to any treatment prior to joining the plan.

Plan membership cannot be combined with other dental insurance plans.
